On September 18th, 2022, Kotoko was eliminated from the CAF Champions League preliminary stage after losing on penalties against Rail Club de Kadiogo. They fell by a goal to zero before sending the game to penalty shoot-outs.

On December 6, 2020, in Accra Sports Stadium, Kotoko played FC Nouadhibou in their first CAF Champions League match. Kotoko won by a score of two goals to zero thanks to goals from Osman Ibrahim and Kwame Poku.
When they defeated FC Nouadhibou to advance from the preliminary round of the CAF Champions League in 2020, they faced AL Hilal Omdurman and suffered their first home defeat under Nana Yaw Amponsah.
With a two-goal deficit to overcome, they returned from Omdurman with Al-Hilal for the second leg on December 23, 2020, but they were defeated at home in the Accra Sports Stadium by a strike from DR Congo’s Vinny Bongonga Kombe in the 76th minute.
